2. Complete this form.
3. Make a copy of your reseller's tax certificate.
4. Make a copy of a voided business check.
5. Fax all material to (440)-323-0648
Upon receipt of the above information, your account will
be upgraded to dealer status and will automatically
receive wholesale prices displayed on our web site
when you add products to the shopping cart.
